Bereavement Needs Phone Assessment
An informal phone interview that allows us to learn more about you and/or your family’s needs.

Orientation & Consent to Services
Grief counselor will review policies and procedures with you and obtain signed consent. This can be completed either in person or via phone. First appointment will be scheduled upon receipt of consent forms.
